If you, as a new or existing member, need to initiate your membership for 2023 (for example, you have not purchased for 2023 as of September 22, 2023), please use the following links to download the paper forms, print, and submit via mail to the MPOA.

These forms are for both new and renewing members. You will also need to renew for the 2024 season (plus assessment), and that can be completed online. First, please complete one of the following forms:
